Hebert Adult Center

The Hebert Adult Center, a Licensed Adult Day Care facility, also called Day Activity and Health Services (DAHS) program, provides daytime services — Monday through Friday — to clients who live in the community. Services address their physical, mental, medical, and social needs. Clients may attend up to five days per week, depending on their eligibility.

Adult Day Care offers an individualized plan of care to meet the needs of each participant who attends on a scheduled basis. All services are administered by our friendly and caring staff.

Full-time Registered Nurse (RN) or Licensed Vocational Nurse (LVN) is always on duty to assure that the participant's private physicians orders are strictly followed.

Services & Activities 

  • Noon meal and snacks
  • Nursing and personal care
  • Physical exercise
  • Social, educational, and recreational activities
  • Transportation to and from the facility, including transportation to and from medical appointments
  • Daily exercise, games, crafts, movies, gardening
  • Educational programs, guest speakers and entertainers
  • Day trips provided weekly

Nutritious Meals

  • Each meal is tailored to individual dietary needs.
  • Nutritious hot lunches and a mid-morning, and mid-afternoon snack.

Eligibility Requirements

  • No age limit for Title XIX-funded services
  • Age 18 for Title XX-funded services.
  • Must have a medical diagnosis and physician's orders requiring care or supervision by a licensed nurse or have a functional disability related to a medical diagnosis.
  • Must need assistance with one or more personal care tasks.
  • Financial eligibility is determined by DADS for Title XX-funded services.

Financial eligibility for Title XIX services is determined by the Texas Health and Human Services Commission for applicants who are not already Medicaid recipients. For more information, please visit their website.
